Please read these Terms of Service carefully before using Lumina Life LLC services. By booking or receiving our services, you agree to be bound by these terms.
1. Services
Lumina Life provides mobile IV therapy services supervised by licensed physicians and administered by licensed practical nurses (LPNs). Our services include hydration therapy, vitamin infusions, and other wellness treatments.
All treatments are provided for wellness and recovery purposes. Our services are not intended to diagnose, treat, cure, or prevent any disease. You should consult with your primary care physician before beginning any treatment.
2. Booking and Payments
Payment is due at the time of booking. We accept major credit cards and other payment methods as specified on our booking platform. Prices are subject to change without notice.
All sales are final unless you cancel according to our cancellation policy.
3. Cancellation Policy
Cancellations made 2 or more hours before your scheduled appointment will receive a full refund. Cancellations made less than 2 hours before your appointment will incur a $75 cancellation fee.
No-shows will be charged the full treatment amount. We reserve the right to refuse service to clients with a history of repeated no-shows or late cancellations.
4. Medical Eligibility
You must complete a medical intake form before your first treatment. You represent that all information provided is accurate and complete. You must disclose:
- All current medications and supplements
- Known allergies
- Pre-existing medical conditions
- Pregnancy or breastfeeding status
We reserve the right to refuse service if we determine that IV therapy is not appropriate for you based on your medical history.
5. Risks and Informed Consent
While IV therapy is generally safe, it carries potential risks including but not limited to: infection, bruising, allergic reactions, phlebitis, and in rare cases, more serious complications.
By receiving our services, you acknowledge that you have been informed of these risks and provide your informed consent to treatment.
6. Limitation of Liability
To the fullest extent permitted by law, Lumina Life LLC, its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of our services.
Our total liability shall not exceed the amount you paid for the specific treatment giving rise to the claim.
7. Insurance and Reimbursement
We are a cash-pay service and do not bill insurance directly. Upon request, we can provide a superbill that you may submit to your insurance company for potential reimbursement. We make no guarantees regarding insurance reimbursement.
8. Service Area and Availability
We currently serve specific areas in South Florida. Service availability may vary based on location, time, and staff availability. We reserve the right to decline service to areas outside our standard service zone or to charge additional travel fees.
9. Intellectual Property
All content on our website and marketing materials, including text, graphics, logos, and images, is the property of Lumina Life LLC and protected by copyright and trademark laws.
10. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any disputes shall be resolved in the courts of Palm Beach County, Florida.
11. Changes to Terms
We reserve the right to modify these Terms at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services after changes are posted constitutes acceptance of the modified Terms.
